HOW TO OBTAIN A BIRTH CERTIFICATE IN BANGALORE


If you're in Bangalore, Karnataka here is  information on how to obtain a birth certificate:
For every child being born, a birth certificate is required by the government of every state and nation. A certification of birth contains important details that accurately identify the person, the information about the name of the parents, the place, date and kind of birth. 
The registration and issue of the certificate of birth and death is done at the following places:
  1. 17 Sub health offices.
  2. Government hospitals.
  3. Bruhat Bangalore Mahanagara Palike referral hospitals
  4. Corporation main office.
Pre-process requirements:
  1. If a birth happens in a hospital, it is the responsibility of the hospital authorities to record the details of birth and inform the Registrar of Births and Deaths. In the case of Bangalore, a Medical Officer, Health (MOH), BBMP performs this role.
  2. At the hospital, the person applying for a certificate should fill a form (Form-1 for births,), which the hospital will then send to the BBMP office. The Medical Officer Health (MOH) here will provide the certificate, which can then be collected at a specified date
  3. The name of the child can be specified in the initial application itself, which would make the procedure easier. Parents can add the name later at the BBMP office before they collect the actual certificate, or they can collect the certificate and then apply for name inclusion any time before the child turns 14 and an updated certificate can be obtained.

  • 1. The Birth certificate is a very important document required throughout one’s life for various purposes. It is the primary source of information for your age proof.
  • 2. Registration of births is mandatory and a very easy process if done within 21 days from the event. From 21 to 30 days after the event, the MOH will certify and collect a fine of Rs 25. After 30 days and within a year, only the Joint Director of Statistics can provide the certificate with a fine of Rs 50 and an affidavit.

HOW DO I GET NEW LPG CONNECTION THROUGH ONLINE - INDIA

Checklist for new connection registration

(Before Registration in web please ensure the following
  • Valid Email ID.
  • Valid Telephone Number.
  • Valid Identification of nearest distributor*.
  • Your household do not posses any LPG/PNG connection
  • At the time of availing connection the following documents shall be provided :-

  • Affidavit in the format that your household do not have any LPG/PNG connection.
  • *Note : In case your locality/ residence does not fall in the area of operation of the distributor selected by you then the distributor may not be in a position to serve you

    HP (HINDUSTAN PETROLEUM):
    The nearest HP GAS distributor may be approached for a new Gas connection. Alternately you may book for new HP GAS connection on- line. Details of cost of New Connection with Double Bottle Cylinder (DBC) (two cylinders and one regulator) along with cost of LPG is given below:

    Fixed Cost: Refundable Deposit for Two Cylinders @ Rs. 1450/ - each = Rs. 2,900/- Regulator Price -(Refundable Deposit for One Regulator) @ Rs. 150/- = Rs. 150/-
    *(For North Eastern States Refundable Deposit for each cylinder is Rs.1150/-.)
    Cost of Two Refills - Present price according to your area as applicable.
    Cost of hotplate/stove and Suraksha Hose (rubber tube) will be extra.

    Stamp duty on actuals is levied wherever applicable.

    FOR ALL DOMESTIC LPG CONSUMERS


    1. Govt. Of India (GOI) has capped supply of subsidised domestic cylinders to 6 cylinders in a year with effect from 14.09.12
    2. For the period of 14th Sept. 12 to 31st march 13 the first 3 cylinders will be supplied at subsidised rate to each domestic consumer irrespective of the number of cylinders consumed up to 13th Sept. 12.
    3. You must ensure that you have your domestic gas consumer card (DGCC) booklet (blue book) with a serial no. And your consumer no. If either book or no. missing you must contact your distributer for a new booklet or entry of no. You will have to produce the DGCC if any dispute arises on supply of cylinders.
    4. The cash memo and acknowledgement generated by the system for each cylinder will indicate the no. of subsidised cylinder being delivered against entitlement i.e. 1/3, 2/3, 3/3 in the current year and 1/6, 2/6, 3/6 etc. in each subsequent year and also indicate the serial no. of DGCC booklet.

    At each cylinder delivery, you must
    ·        Produce the DGCC booklet to the delivery man while receiving the refill cylinder.
    ·        Check the serial no. of the DGCC matches with the printed on the cash memo.
    ·        Sign on the DGCC booklet as well as copy of the cash memo as acknowledge of receipt of the refill cylinder and handover the same to delivery man.
    ·        Ensure that the delivery details are recorded in the DGCC book and that should be signed by the delivery man.
    ·        Keep the cash memo and DGCC book carefully as in case of any dispute with regard to receipt of subsidised cylinders, the cash memo and DGCC book will have to be produced for any claims.
    ·        You are requested to register your mobile no. with the distributer so that confirmation of delivery of the refill cylinder can be sent through SMS by the respective oil company to their consumers as and when electronic confirmation system rolled out.
    ·        The LPG prices except subsidised domestic LPG cylinder is likely to change every month subject to change in international price of LPG

    You are also advised to check your booking and delivery details on transparency portal of oil companies ( www.petrolrum.nic.in / www.indane.co.in / www.ebharatgas.com / www.hindustanpetroleum.com )and in case of any discrepancy in delivery of cylinders, immediately log complaints in the portal.
